Medical Care at Home Comes of Age

Over the last 30 years, a variety of home-based medical care models have been developed and implemented to address important gaps in health care delivery, especially for people with multiple chronic conditions and functional impairments. These models are becoming increasingly important as patients seek care that is person centered and meets their complex needs. This is especially true in the context of the ongoing COVID-19 crisis, which has drawn into sharp focus the need for care models that go beyond the traditional “bricks and mortar” of physician offices and hospitals.
